Camp Health Aide Program Sample Grant Proposal
Migrant Health Promotion has adapted this sample proposal from one of our successful grant proposals. The sample proposal includes a brief needs statement, program plan, objectives, timeline, evaluation plan and references.
In this grant proposal, Migrant Health Promotion requests $50,000 to conduct a 13-week Camp Health Aide Program focused on improving health care access. The proposal describes how the program will serve farmworker families in Monroe and Lenawee Counties of Michigan in the summer of 2004. As proposed, eight Camp Health Aides will receive a stipend to dedicate at least 20 hours per week to the program. They will participate in training, receive individual supervision and serve their community by carrying out at least ten individual encounters per week with labor camp residents. They will also facilitate at least three group health information sessions during the 13-week program. As a result, 320 farmworkers will have increased access to health care.
